Nursing Facility Transition Coordinator -Bottineau, ND

The primary responsibility if this job is to assist people with disabilities to transition from a Nursing Facility back into the community and to provide follow up case management after the transition. 

This position will provide services in Renville, Bottineau, Roulette, Pierce and Mchenry counties.  A preference will be given to those with a licensure as a Licensed Social Worker (LSW) by the ND Board of Social Work Examiners, however, we will consider those with other related degrees and college experience. Applicants should have  and at least one year of  related professional human services employment experience.
 
The provision of the above outlined services will be provided in accordance with the current Nursing Facility Transition Process Guidelines established by the MFP Nursing Facility Transitions Workgroup. Must have good organizational, planning, communication, and interpersonal skills, and work well as the member of a team. Individuals must be able to exercise diplomacy, tact, and good judgment. 
  
This job 40 per week with a competitive benefit package. 


Independent Living Specialist-Minot 

We are looking for an Independent Living Specialist to work out of our Minot, ND office. The primary function of this job is to provide information and referral, advocacy, skills training and nursing home transition services to people with disabilities of all ages. This job requires you to able to work well independently and be able to learn complicated tasks quickly. The ideal candidate will possess the ability to communicate well with people from various social and ethic backgrounds both verbally and in writing.  


This job 40 per week with a competitive benefit package.
